Your Opportunity
The Housing Accommodation Support Initiative (HASI) is an adult mental health recovery program that provides complex case management and direct support in the community for individuals who are at risk of repeated hospitalisation and/or homelessness.
* Provide psychosocial support to consumers clinically assessed as having severe mental illness, with high levels of psychosocial impairment to increase skill, knowledge and independence regarding activities of daily living such as tenancy maintenance.
* Assist consumers in connecting and accessing community-based social activities.
* Provide psycho-education and support to manage mental distress regarding experiences of mental health.
* Coordinate and attend appointments with consumers to support psychosocial recovery.
* Assisting consumers in the creation, implementation, and review of holistic person-centred support plans through a range of activities.
* Provide comprehensive community care coordination and advocacy.
* Develop and maintain relationships with internal and external stakeholders including service partners, government agencies, and external supports.
* Assist in the promotion and awareness of HASI and its involvement in the community activities.
* Assist consumers to improve their quality of life and to enhance their skills to continue living independently in the community.
Requirements for Success
* A relevant tertiary qualification - min of Cert IV in Mental Health.
* Relevant work experience in Mental Health or community services-related environment.
* Knowledge and understanding of recovery-oriented practices.
* Strong communication and relationship building skills.
* A willingness to learn and develop your skills.
* Drivers licence and high level confidence driving in a wide catchment area.
* Must be able to work evenings and weekends.
