Evidence suggests that people who re-integrate into the community following a period of imprisonment require transitional support in order to reduce their risk of reoffending.

Advanced Practitioners manage a caseload of complex and high-risk individuals on court orders, many of whom have completed part of their sentence in custody.
They draw on evidence-based frameworks to deliver dynamic case management practice with people who have complex needs and those who have committed serious offences.
You will help a caseload of complex and high-risk clients meet their court order conditions and connect them to community programs and services.
You will also lead and mentor staff, guiding them to deliver quality evidence-based case management practice to clients.
Some of your day-to-day responsibilities include: preparing case plans providing interventions coordinating service providers and care teams addressing the underlying causes of offending leading and developing case management staff.
