The role:
This is a permanent part-time position starting with 3 days per week with the possibility to increase to 5 days. The role involves travelling between customer’s homes and providing individual and personalised support to customers in accordance with their support plans and/or medical practitioner’s directions.
Duties and responsibilities may include:
1. Working with or under the guidance of medical practitioners and Allied Health professionals
2. Medication Administration and Complex Clinical Care e. palliative care, bowel care/suppositories, catheter care, stoma care, oxygen and more
3. Monitoring and communicating on the customer’s wellbeing and reporting on progress of customers
4. Ensuring that all services are provided in a caring and respectful way in accordance with Just Better Care’s policy and procedures, legislative requirements and program specific guidelines (i.e. DVA Community Nursing)
5. Providing training, coaching and guidance to Community Support Professionals, Enrolled Nurses and Support Staff/Case Managers as required
6. Conducting skills assessments of Community Support Professionals performing complex care services as required
7. Demonstrating a commitment to best practice and evidence of continued professional development
8. Participating in staff meetings and attending training opportunities when required
What you will need:
9. Current CPR / First Aid Certificate
10. Current and satisfactory Federal Police Check, NDIS Worker's Screening Card & Working with Children Check Blue Card
11. Current Australian Driver’s Licence
12. Proof of Working rights in Australia (e.g. Passport, Birth certificate)
13. Reliable, registered and insured vehicle (comprehensive insurance is preferred)
14. Smartphone with internet access
15. Registered AHPRA
16. COVID-19 Vaccination Certificate
