The Integration Officer Aged & Palliative Care position supports the strategic objectives of Western NSW Primary Health Network (WNSWPHN) to lead support and strengthen person centred primary health care, by developing and coordinating activities to related to older people's health, aged care and palliative care. This role will focus on the management, needs assessment, commissioning, implementation, and coordination of WNSW PHNs general practice in aged care (GPAC) response, and related activities. Selection Criteria Essential Experience in project management, including creating and managing project timelines. Demonstrated understanding of Aboriginal culture including a commitment to cultural awareness and safety. Demonstrated high-level interpersonal and communication skills and high-level competence in using information communication technology. Demonstrated organisation and time management skills, with proven ability to deliver high quality outcomes in agreed timeframes.Capacity to travel within western NSW; hold a current driver's licence.
