We are seeking a compassionate and experienced Registered Nurse (RN) and Enrolled Nurse (EN) to join our dynamic team as we provide exceptional community care services including Aged Care. The ideal candidate will have previous aged care, hospital, and community/home care experience and be proficient in catheter care. Responsibilities:
* Provide high-quality nursing care to patients in the community setting, including assessments, medication administration, wound care, and catheter care.
* Collaborate with interdisciplinary team members, patients, and their families to develop and implement comprehensive care plans that meet patients' individual needs.
* Educate patients and their families about health management, disease prevention, and self-care techniques.
* Monitor patients' condition and response to treatment, and make appropriate adjustments to care plans as needed.
* Maintain accurate and up-to-date documentation of patient assessments, care provided, and patient outcomes.
* Act as a resource and mentor to other team members, providing guidance and support as needed.
* Adhere to all relevant policies, procedures, and standards of practice, including infection control protocols and safety regulations.
Qualifications:
* Current and valid Registered Nurse (RN) and Enrolled Nurse Registration.
* Minimum of 6 months of either hospital nursing experience or aged care/community care experience.
* Knowledge of ACFI and aged care quality standards.
* Excellent communication and interpersonal skills to effectively interact with patients, families, and interdisciplinary team members.
* Ability to work independently and manage time effectively in a community care setting.
* Strong critical thinking and problem-solving skills to assess patient needs and make appropriate clinical decisions.
* Familiarity with electronic health records (EHR) and other relevant healthcare technology.
* Valid driver's license and reliable transportation for home visits.
* Ability to travel for work at multiple sites.
