About The Role
MyHealth, Amplar Health and Medibank have teamed up to launch an exciting new pilot program, designed by GPs, for patients, and supported by a diverse healthcare team. This innovative program focuses on improving patient outcomes in general practice through proactive care and multidisciplinary case management and care coordination for patients at risk of and with chronic and complex health conditions.
What's new about it?
We are trialling a fresh approach to patient care with the following goals:
Preventive Health - Instead of reacting to health issues, we're focusing on proactive management of patients who are at risk of hospitalisation and long-term health issues. Case Coordination & Collaboration - With a focus on team-based care, our new model integrates GPs, nurses, pharmacists, social workers, and healthcare staff for better patient outcomes. Social Determinants of Health - Recognising health is determined by a wide range of factors and issues, our model has a focus on identifying and supporting patients with social wellbeing needs.
We're looking for Accredited or Consultant Pharmacists to join this innovative program. As a pharmacist, you'll play an essential role in case conferences, conducting home medication reviews, and helping patients to manage their chronic diseases through patient education and collaborative care with GPs, nurses and social workers.
The Role
Flexible & Hybrid: Enjoy a hybrid work model with minimal clinical hours, + flexibility to virtually participate in case conferences, conduct medication reviews and patient education in the patient's home, our clinics, and via telehealth. Contract: 1-year contract (with potential for extension), offering flexibility and the opportunity to be part of a pioneering healthcare program. Proactive Patient Care: Focus on identifying opportunities for medication optimisation, better patient adherence, health screening, and self-management of health. Collaboration: Work alongside GPs, other healthcare professionals, and the patient's broader support team. Education: After reviewing a patient's medications, provide valuable education on their health and medication regimen.
Key Responsibilities
Participate in case conferences and collaborate with the team on care management. Conduct home medication reviews Provide medication reviews in clinic and via telehealth Drive medication optimisation for patients Deliver patient education and support with patient self-management and medication use and adherence. Identify and follow up on opportunities for proactive and preventative health interventions. Supporting improvements in clinical practice Collaborate with community and hospital pharmacists, where required Conduct patient review post hospital discharge, where required
What We're Looking For
Essential
Tertiary qualification in pharmacy with current registration as a pharmacist with the Australian Health Practitioner Regulation Agency (AHPRA) Medication Management Review Accreditation Minimum 3 years' experience in conducting medication reviews. Strong communication and interpersonal skills including the ability to influence and facilitate change The ability and the enthusiasm to work independently and as part of a team Experience delivering patient education for medication management and preventative health A passion for patient care and innovation in chronic disease management.
Preferred:
Prior experience working in multidisciplinary teams, particularly in GP-led environments. May hold other certificates or be working toward other relevant qualifications. Examples may include but are not limited to postgraduate clinical pharmacy, diabetes educator, asthma educator. Familiarity with medical practice management software and relevant computer applications Knowledge of Medicare, DVA, Work Cover, HICAPS, and Private Health Insurance billing preferred
