ONCALL is currently seeking an Enrolled Nurse to join our team as a Clinical Support Officer and ensure the effective assessment, planning, and coordination of client support services while maintaining high standards of data management, compliance, and stakeholder engagement.
In this key role you will:
o Conduct comprehensive client assessments, which may involve visiting clients at their residences.
o Develop and maintain individualised client support plans that align with their needs and organisational guidelines.
o Collaborate with service delivery teams to ensure the effective implementation of support plans.
o Organise, collate, and upload client documents from various sources, and provide reports on document management progress.
o Act as the primary contact for document-related queries and ensure seamless communication between departments and any relevant external providers.
o Deliver training and ongoing support to team members on best practices in care planning and document management.
To be successful you will need:
o Registration as an Enrolled Nurse
o Relevant qualification in Health, Social Work, Administration, or a related field.
o Experience in client assessment, care planning, or a similar role within a healthcare or community services setting.
o Strong background in data management, compliance, or quality assurance.
o High-level proficiency in data entry and record management systems, including Visualcare, SharePoint, or similar platforms.
o Strong knowledge of compliance regulations related to data protection and client confidentiality.
o Driver’s Licence.
o Relevant industry background checks, or willingness to obtain.
