About the Opportunity
In response to the COVID-19 pandemic, the Victorian Government’s Homelessness to a Home initiative enabled access to approximately 1,800 homes to support those experiencing homelessness who are residing in emergency accommodation due to COVID-19, providing stable housing and support services. The Homelessness To Homes support model includes a suite of primary and allied health, specialist housing, assertive outreach, therapeutic supports, early intervention & prevention services, legal, education and employment support. Our therapeutic supports allow us to respond and creatively engage those people with the most complex barriers. We are currently recruiting for a skilled and enthusiastic Homelessness to Homes Case Managers who are responsible for the provision of high quality intensive goal directed support and case management services to people experiencing homelessness who may also have complex needs and have become part of the Homelessness To Homes program across both the Western and Northern regions of Melbourne. People will be provided intensive and holistic support to achieve housing stability and broader goals.
About you
We are looking for someone who can:
4. Effectively manage a caseload through a healing-oriented framework. Utilising creative and innovative pathways to disrupt disadvantage and create pathways out of homelessness.
5. Develop advanced goal directed case planning and crisis interventions to meet the needs of people presenting with multiple and complex needs, including risk assessments and safety plans.
6. Assist people with complex needs to access ongoing accommodation options, particularly supported accommodation, family reconciliation, private rental, shared housing and social or transitional housing in accordance with the caseplan.
7. Facilitate access within the multi-disciplinary team to relevant specialist support services, including health, mental health, AOD and therapeutic interventions through consortium and where appropriate, external services in accordance with the caseplan.
8. Maintain up to date knowledge of MCM’s programs and relevant external service providers to ensure an integrated service response to people.

Essential:
13. A bachelor qualification in social work, youth work or a related tertiary qualification, or extensive experience working in the community services sector
14. Demonstrated experience working within a case management framework.
15. Understanding of mobile, street based and assertive outreach support and interventions.
16. Understanding or experience working with people at risk or experiencing homelessness, including knowledge of healing-oriented approaches, and a strong ability to actively and assertively engage young people with complex needs.
