The Opportunity Our Client's Home Health team is on the lookout for passionate Case Coordinators to support our SA Community Care and My Home Hospital programs that delivered to patients in their home. · Using your clinical skills, you will ensure a comprehensive client centred management plan is achieved for our clients together with: · Conducting clients initial phone assessment, care planning and goal setting. · Coordination of in-home services to support the rehabilitation and recovery of clients · Building and maintaining effective relationships with a variety of different stakeholders including primary and secondary care organisations and your non-clinical support team · Monitoring patient progress and updating care plans as appropriate · Liaison with healthcare professionals and specialists · Discharge planning including referrals to long term services · Provision of telephone support, education, and advice The Case Coordinators work a rotating 7-day roster between the hours of 7am to 10pm. You will be rostered to work alternate weekends in the comfort of your own home. There is also an on-call component to the role. We are currently seeking RNs who are able to accommodate part time or full time from 0.6 (22.5 hours per week) to 1.0 FTE (37.5 hours per week). This is a permanent position. It is important that you can commit to work 3 day in a row if part time About You As an outstanding communicator, your intuitive people skills will be central to your success. You will be naturally empathetic, observant, attentive, trustworthy, and committed to making your patient feel safe and comfortable. You will be client and goal focused who is driven to achieve positive customer outcomes. You will also have: · Minimum 2 years' experience as a Registered Nurse with unrestricted AHPRA registration · Recent experience in care coordination. · Experience working in hospital setting is also desirable. · Strong customer service orientation and relationship management · Excellent clinical reasoning skills · Ability to work in a fast-paced environment. · Proficient computer skills to enable fast and accurate data entry into patient electronic medical records.
