They are looking to fill a position for a diverse role comprising OT-specific assessments, interventions and planning predominantly in the NDIS framework to increase people’s function and independence in all aspects of their lives. The position of an Occupational Therapist is to provide high quality, effective and efficient service to clients accomplished by working as a member of the integrated healthcare team with a focus on providing person-centred, holistic and goal-directed care that promotes clients’ wellness and active participation in decisions about their care.
About the Role:
It is a community OT role managing a mixed caseload across the lifespan within the NDIS framework.
Key Responsibilities:
o Caseload Management: Maintain responsibility for a client caseload, ensuring alignment with PHASE policies and funding requirements.
o Occupational Therapy Services: Perform OT-specific assessments, including home safety, functional, and case management for NDIS participants, aimed at enhancing capacity, function, and social engagement across various settings.
o Home Modifications: Assess, plan, and manage minor and major home modifications to support accessibility and independence.
o Assessments and Reports: Conduct functional, pre-employment, ADL, workplace, scooter, and vehicle modification assessments. Provide timely written and verbal recommendations and reports.
o Equipment Prescription: Recommend and prescribe equipment and ongoing therapy support tailored to client needs under NDIS and other schemes.
o Collaborative Care: Liaise with clients, families, referrers, healthcare professionals, and other stakeholders to deliver coordinated, capacity-building outcomes.
o Health Promotion: Advocate for health and wellness, supporting mental and physical well-being through targeted solutions.
o Professional Engagement: Attend interdisciplinary health meetings and provide expert advice to workers when required.
o Mentorship: Train, mentor, and support the development of students within the field.
o Education & Prevention: Deliver health education to clients, carers, and professionals as needed.
o Professional Development: Stay current with industry practices through literature, education, supervision, and interest groups.
o Client Services: Perform client-related activities, including home and community visits, while adhering to NDIS competency frameworks.
o Administrative Tasks: Manage scheduling, data entry, information reporting, and database maintenance using CRM (MYP).
o Caseload Monitoring: Effectively manage participant case ratios, monitor hours of service against agreements, and ensure service delivery aligns with client goals.
o Business Contributions: Participate in staff meetings, training, and community events to promote PHASE as a leading NDIS service provider.
o Regulatory Compliance: Develop a deep understanding of NDIS legislation and ensure compliance during audits and service delivery.
o Quality Improvement: Contribute to allied health projects, service delivery enhancements, and quality improvement initiatives.

Experience/Knowledge
o NDIS Expertise: Strong working knowledge and understanding of the NDIS framework.
o Integrated Healthcare Experience: Demonstrated experience in integrated health, allied health, and primary care, with the ability to collaborate effectively in multidisciplinary teams to achieve agreed deliverables and outcomes.
o Self-Management and Organization: Proven ability to organize and proactively manage workload to meet annual performance targets and service objectives.
o Decision-Making and Customer Service: Sound judgment and decision-making skills, with a strong commitment to delivering exceptional customer service.
o Inclusive Mindset: Positive attitude and approach towards supporting individuals with disabilities.
o Communication Skills: Excellent interpersonal, written, and verbal communication skills, with the ability to engage effectively with internal and external stakeholders at all levels.
o Diverse Client Experience: Experience working with children and adults with disabilities or mental health challenges, including those from culturally and linguistically diverse backgrounds.
o Teamwork and Independence: Ability to work independently while also contributing effectively to team goals.
o Sector Experience: Previous experience in Allied Health, Aged Care, Therapy, or Disability service settings.
o Technical Proficiency: Competency in using databases, software, and the MS Office suite.
o Task Efficiency: Demonstrates the ability to efficiently and effectively complete assigned tasks within deadlines.
o Safety Awareness: Maintains a strong focus on safety protocols and preventative measures.
