Barwon Health is committed to developing a vibrant culture of education, training and research that advances health and wellbeing for all.About the Role: The Cancer Nurse Consultant – Geriatric will be working with the Regional Trials Network in support of the Geriatric Initiative of a MRFF funded project (ReViTALISE). The Cancer Nurse Consultant – Geriatric will develop a model to introduce Geriatric Assessment within Cancer Services at Barwon Health and ensure its sustainability moving forward. The Cancer Nurse Consultant Geriatric works in partnership with multi-disciplinary teams to contribute to the continuity and quality of care for patients diagnosed with Cancer. The role provides individual coordination and support to patients and their families. The level of intervention by the coordinator will vary according to the complexity of care required, based on individual patient needs in their care. Workload will be self-generated, achieved through extensive interdisciplinary collaboration across primary, secondary and tertiary health care providers.
