About the Role
AHP-2
As a Psychologist / Clinical/Health Psychologist within the CALHN Chronic Heart Failure Self-Management Program, you’ll be instrumental in providing specialised psychological care to adults managing chronic heart failure. You’ll conduct thorough assessments and design tailored intervention programs, offering direct support to patients coping with heart failure-related psychological challenges. By promoting health and wellbeing practices, you will help patients improve their disease self-management and overall quality of life.
Supported by a multidisciplinary team, you’ll play a key role in planning, delivering, and evaluating the Chronic Heart Failure Psychology Service, ensuring that each patient receives evidence-based, compassionate care. You will also have the opportunity to contribute to research and quality improvement initiatives, helping to advance the service’s approach to chronic heart failure management.
AHP-3
As a Senior Clinical/Health Psychologist within the CALHN Chronic Heart Failure Self-Management Program, you will provide specialised psychological care to adults managing chronic heart failure. Utilising your clinical expertise, you will conduct thorough assessments and individually tailored interventions, working directly with patients to address Chronic Heart Failure-related psychological challenges and to improve disease self-management. Your role will be pivotal in fostering positive health and wellbeing practices within a multidisciplinary setting.
In this leadership position, you will oversee the planning, delivery, and evaluation of the Chronic Heart Failure Psychology Service, incorporating both individual and group therapies. Additionally, you’ll lead research and quality improvement initiatives, ensuring evidence-based care continues to evolve within the service. As a clinical leader, you will provide supervision to provisionally registered Psychologists on clinical placement and support other psychologists within the CALHN psychology team. This role is an exceptional opportunity to lead, innovate, and make a lasting impact in chronic disease management.
