About the role
This role will provide leadership as a member of a multidisciplinary team to clients referred for Aged Care Assessment. There is a requirement to deliver high quality social work services through the provision of comprehensive assessment and knowledge of relevant services to promote, maintain and restore the health, functional ability, and quality of life for older adults.
Some of the key accountabilities of the role are:
* Demonstrated competence and experience in delivering social work in accordance with the Australian Association of Social Work (AASW) Code of Ethics and Professional Practice utilising contemporary evidence-based practice principles.
* Apply knowledge and skills in assessment and provide targeted interventions as part of a team to achieve positive outcomes in relation to client identified goals.
* Apply a high degree of independent clinical judgement and work autonomously as a part of a responsive approach to identified clinical need or interventions relevant to unmet care needs of client.
* Demonstrated effective verbal and written communication skills including client assessment, interdisciplinary communication, correspondence with all stakeholders, clinical reports consistent with medicolegal documentation requirements.
About you
Our ideal candidate will be someone who can demonstrate the following:
* Competency in My Aged Care comprehensive assessments is highly desirable though not essential.
* High quality Social Work advice to assist transdisciplinary care by effectively integrating the social work perspective and collaboration as required with government and non-government service providers to improve health outcomes.
* High level expertise and skills in delivering assessment, intervention and independent decision-making reflecting evidence informed interventions to adults to ensure the maintenance of social work practice standards.
* High level written and oral communication skills in providing multidisciplinary care in a complex environment including psychosocial assessments, management of clinical documentation and reports to address client needs and high-level advocacy to ensure access to services.
* High level professional leadership skills using a solutions focussed approach applied to the current and potential clients of the program engaging cross discipline involvement where appropriate, whilst exercising a high degree of independence and professional decision making.
* High level of clinical leadership in the supervision and development of staff including supporting effective and highly accountable social work practice and active ongoing professional development.
