SWAMS is seeking dedicated and compassionate Community Support Workers who have completed their Certificate III in Individual Support to join our casual pool of Support Workers in Mandurah.

Our mission is to provide high-quality, culturally appropriate, and holistic healthcare that builds strong and healthy Aboriginal communities.
About the Role:
The Community Support Worker will play a vital role in providing assistance and support to individuals within the community. This position requires a compassionate and empathetic individual who can effectively communicate and build relationships with the Aboriginal people in our area. The role includes:
* Working actively with clients as part of the aged care and community services team.
* Implementing programs to enhance client skills, independence, and community participation.
* Encouraging personal and social relationships among clients.
* Liaising with elderly clients to promote engagement in home support and respite activities.
* Encouraging Elders to follow recommended referral processes for specialists and health checks.
* Fostering collaborative relationships with other health service providers.
Minimum Requirements:
* Certificate III in Individual Support.
* Demonstrated experience in working in aged and community care services.
* Sound knowledge of the aged care and community sector, understanding of Aged Care Client Rights, Aged Care Accreditation Standards and Outcomes.
* Knowledge of health and well-being issues affecting Aboriginal and Torres Strait Islander people.
* Proven ability to work both independently and as part of a team.
* Ability to work autonomously and display initiative, self-motivation, time management and solution-focused skills.
* A certificate or tertiary qualification in Community Services, Aged Care or similar field.
* Current knowledge of legislative obligations for Equal Opportunity, Disability Services, Occupational Safety and Health.
