The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth). Our Child and Family Health team is seeking to engage Registered Nurses. The Registered Nurse will ensure provision of a Universal Health Home Visit (UHHV) to all families of newborn babies. This includes monitoring the growth, development and health status of the child (0 – 5 years) within the context of the family by using child health surveillance and screening as per the Personal Health Record (Blue Book). As a Registered Nurse, you willl ensure optimal outcomes for clients and carers of the service through the promotion of best practice including the appropriate escalation of care. The role will allow you to work collaboratively utilising effective team communication with members of the multidisciplinary health team in the planning, implementation, assessment and evaluation of client care. The successful incumbent will act as an advocate for the specific needs of all clients and carers including those from the indigenous population or other multi-cultural groups providing appropriate care to the client undergoing specialised procedures. What you will bring to the role: Current registration as Registered Nurse Division 1 with AHPRA Demonstrated recent experience and clinical skill in provision of care to children aged 0-5 years and their families/carers as a registered nurse (including completion of a new graduate RN program or 12 months full time equivalent experience working as a Registered Nurse) Demonstrated evidence of working within a Multidisciplinary team to facilitate the care planning for families with complex needs. Demonstrated effective communication (written and verbal) and interpersonal skills Demonstrated ability to organise and prioritise work and an ability to effectively work both within a team and autonomously Demonstrated commitment to professional development, evidence based practice and an awareness of changes within the health care environment Demonstrated knowledge of and ability to utilise information technology systems within a variety of applications (e.g. Cerner) and commitment to undertake training in electronic health record systems as required Current Driver's Licence and willingness to use for work purpose travel.
