What you'll be doing
This role is integral to ensuring that our patients receive exceptional, continuous care in the comfort and safety of their homes. This position requires a commitment to a flexible work schedule, including availability during weekends and public holidays, reflecting our dedication to providing continuous care seven days a week. As a key component of the District's Virtual Care and Hospital in the Home Services, this position entails collaborative efforts with a diverse range of multidisciplinary team members, encompassing medical, nursing, and allied health professionals. The initial focus will be on the further development of the service at Hornsby Hospital, enhancing our capacity to deliver exceptional care across the District. While this is predominantly an in-person role based at Hornsby Hospital, there may also be requirements to work across other sites within the District.
Purpose of the Position The primary purpose of this role is to provide high quality clinical care of patients using a hybrid model of both face-to-face assessments ( hospital, clinic, home visit) and telehealth ( video, phone, remote patient monitoring). In addition, the role is to assist the General Manager and Clinical Director with the planning, development, implementation and delivery of a broad range of innovative models of care in both the Virtual Care and Hospital in the Home settings.
Provide clinical management and treatment of patients under your care in a Northern Sydney Local Health District (NSLHD) facility or service. This care will be provided within the scope of practice endorsed by the NSLHD Medical and Dental Appointments Advisory Committee (MDAAC).
The position will provide clinical consultation services within NSLHD and will contribute to the teaching and training of medical students, graduates and other health care professionals and to research, evaluation and quality improvement activities within the District.
