Join our Social Work team and provide support to our PSOS and Therapy communities.We have a rare and fantastic 9 month maternity leave cover for a Social Worker to provide evidence based and consultative social work support across both our Paediatric Spinal Outreach Service (PSOS) & our therapy services, managing communication and connectivity between internal and external stakeholders.Our PSOS and therapy services are state-wide across NSW, with PSOS delivered as an outreach service, offering specialised multidisciplinary intervention to children and adolescents (0-18) who have acquired a spinal cord injury. The service is a collaboration between Northcott and the SpineCare Foundation and is funded by The Sydney Children’s Hospital Network.You will also engage regularly with our multidisciplinary therapy teams consisting of Speech Pathologists, Occupational Therapists, Physiotherapists, Dietitians and Behaviour Support Practitioners who provide evidence informed and contemporary therapies for our customers in centre, community and schools.We support customers with a broad mix of diagnoses, including autism, intellectual impairment, chromosomal & genetic conditions, as well as acquired & congenital physical disabilities.We have a multidisciplinary and interprofessional approach, enabling our therapists to work together so our customers can get the most out of their therapy and reach their goals successfully.The position can be based at our North Parramatta office, or other Northcott Metro Sydney locations by agreement, with travel across NSW as needed for PSOS. We provide company vehicles for travel but you may also be required to use your own vehicle at times, with appropriate reimbursement if this is required.We envisage the position to be worked across 5 days a week, split between PSOS 3 days, and Therapy 2 days. Though, for the right candidate wanting a 9 day fortnight, the split can be discussed.KEY RESPONSIBILITIES/DUTIES OF THE POSITION:Conduction of high-level psychosocial assessments and development of client focused support plans.Provision of prospective planning for key transition points (e.g. school, surgery and vocation).Work within a Strengths Based, Family Centred approach.Ensure children and young people have access to positive developmental experiences.Support families to access mainstream and specialist services to meet long-term needs.Build networks and working relationships with rehabilitation teams of Children’s Hospitals, relevant community services and government services.Provide outreach social work intervention within the client’s home, school and or other relevant context.Provide casework support including; service coordination, rehabilitation planning reports, organising and attending appointments; psycho-education, practical assistance, advocacy and referral.Close liaison with compensation scheme providers including the NSW Lifetime Care and Support Authority (LTCSA), Compulsory Third Party Insurance Scheme (CTP), Trustees and the National Disability Insurance Scheme (NDIS).Provide counselling to facilitate emotional adjustment to injury.Conduct student specific education sessions at schools and other relevant facilities.Source updated information of new services, funding changes and research relevant to this client group.Participate in relevant internal and external meetings.Maintain accurate clinical documentation, and prepare formal reports, rehabilitation plans and other related correspondence.Provide mentoring and support to Northcott therapy teams, and coordinators in Community Programs and respite services, particularly in relation to complex case management and life-limiting conditions.Provide support to our clients and their families across all therapy centres, as well as clients in our Community Programs, and clients accessing short and medium-term accommodation, through provision of an inclusive and client centric experience.KEY REQUIREMENTS FOR THE POSITION:Degree in Social Work and membership of the AASW.Post-graduate experience in the provision of service to people with a disability/complex needs.You have a strong motivation for achieving workplace targets as appropriate for your level of experience.Experience in working with families in crisis and/or experiencing grief and loss & working with children with physical disabilities and their families, preferably within a community setting is desired.Experience with spinal cord injury desired.Current NSW driver’s license and willingness to travel as required.Experience in working within clinical governance, quality improvement and risk management frameworks.Highly developed skills in conducting psychosocial assessment and support plan development.Experience in leading critical reflection, applying theoretical frameworks and referring to research and literature to inform clinical practice.Experience preparing and delivering presentations & training for teachers, carers, medical professionals and internal stakeholders.Excellent customer relationship management and a proven ability to manage customer service across multiple stakeholders within an organisation.Work collaboratively with colleagues in a multi-disciplinary services environment.Experience in mentoring of staff.Availability for on call support on occasion.Current Working with Children Check and NDIS Worker Screening Check.Great things about working with Northcott:You will be joining our friendly supportive and collaborative teams across NSW, QLD and the ACT.Salary to increase your take home pay up to $15,900.Family friendly workplace with flexible hours to manage home and work lives effectively.Negotiable ability to work from home/RDO’s when appropriate to your caseload.Paid Professional Development leave of 10 days pro rata including some funding towards approved courses, seminars, etc.Receive peer to peer support, formalised supervision, and attend regular professional forums.5 weeks of paid annual leave and paid maternity leave.Multiple employee benefits, such as discounted gym & health fund membership, discounted motor vehicle services and entertainment cards.For a confidential discussion about this position please contact:Janelle Delos Santos, Service Manager 0479 026 417.Northcott offices will be closed for the Christmas break from 22/12/2023 and will reopen on Tuesday 02/01/2024.
