Covering an area of over 1700 km2 from Sunbury to beyond the borders of Macedon Ranges Shire our 200 staff and 70 volunteers are based across four sites at Kyneton, Romsey, Sunbury. About the role We have a unique opportunity available to work as a Diabetes Educator at Sunbury Cobaw Community Health on a casual basis. The position is open to someone who is credentialed or working towards credentialing and has demonstrated experience in diabetes assessment and management. There will also be the opportunity to facilitate Diabetes education and exercise sessions with a Exercise Physiologist. You will have great interpersonal skills and the ability to work both autonomously and within multidisciplinary teams to provide education and support to people living with diabetes in the Sunbury and Macedon Ranges Region Key Duties & Responsibilities • Conduct initial assessments and reviews for adult clients referred for diabetes education. • Practice evidenced based models of chronic disease management practice such as client directed goal setting, motivational interviewing, and health coaching, to educate and promote healthy lifestyle options for those at risk of developing Diabetes as well as adults already diagnosed with Diabetes. • Conduct other appropriate interventions for clients which may include clinical nursing management or advocacy to other health professionals • Acting as a key worker for diabetes management, coordinate client care by consulting, liaising and making referrals to other members of the care team, including endocrinologists, general practitioners, other primary care provider/s and relevant community services • Plan, promote, facilitate and evaluate A “Don’t Sugar Coat Diabetes’ program at regular periods throughout the year for existing and new clients to whom the group educational and peer support model is considered valuable. About you • Ability to build respectful collaborative partnership with key community stakeholders. • Ability to establish and build mutually beneficial partnerships and alliances with current and potential service delivery partners. • Demonstrated personal resilience and ability to manage change or other challenging situations. • Ability to work with a high degree of autonomy and as part of a multidisciplinary team. • Effective leadership skills including highly developed interpersonal skills, communication, problem solving, conflict resolution and negotiation.
