We are seeking a dedicated and compassionate Community Nurse (Registered Nurse) to join our healthcare team in Melbourne. This role involves delivering high-quality nursing care to clients in their homes and the community, supporting individuals with diverse healthcare needs, and promoting independence and well-being.
As a Community Nurse, you will work autonomously and collaboratively within a multidisciplinary team to provide holistic care, health education, and support to clients, including elderly individuals, people with disabilities, and those recovering from illness or surgery.
Key Responsibilities
1. Deliver person-centered nursing care in clients’ homes or community settings in accordance with Australian healthcare standards.
2. Conduct comprehensive health assessments, develop individualized care plans, and provide ongoing monitoring and evaluation.
3. Administer medications, wound care, catheter care, injections, and other clinical procedures as required.
4. Educate clients and their families on health management, disease prevention, and lifestyle modifications.
5. Support clients with chronic disease management, rehabilitation, and palliative care.
6. Collaborate with general practitioners, allied health professionals, and other healthcare providers to ensure integrated care.
7. Maintain accurate clinical records and documentation in compliance with healthcare regulations.
8. Adhere to infection control protocols and workplace health and safety guidelines.
9. Participate in case conferences, care planning meetings, and continuous professional development activities.
10. Provide emotional support and advocacy for clients to enhance their well-being and independence.
Requirements
1. Registered Nurse qualification (Bachelor’s Degree in Nursing or equivalent).
2. Current AHPRA registration (or eligibility to obtain registration).
3. Minimum of 2 years of relevant experience, preferably in community, aged care, home care, or primary health settings.
4. Strong clinical assessment, wound care, and medication administration skills.
5. Valid Australian driver’s license (or ability to obtain one) and access to a reliable vehicle.
6. Excellent communication and interpersonal skills.
7. Ability to work independently and within a team in a community setting.
8. Understanding of Australian healthcare standards, infection control, and patient safety guidelines.
