About you:
In your new role you will…
1. Demonstrate high level Aged Care knowledge (including dementia), with high level knowledge, skills and experience in conducting comprehensive assessments for complex client presentations.
2. Demonstrate ability to provide leadership, precept staff and contribute to professional allied health/nursing practice.
3. Demonstrate high level communication and interpersonal skills, with the ability to liaise and consult with clients and their families, as well as key partner agencies.
4. Demonstrate high level organisational skills, including the ability to effectively prioritise assessment caseloads, while ensuring timely service provision and meeting service timelines and KPIs.
About your opportunity:
In this position you will deliver high level clinical assessment completing comprehensive assessments of frail older people and their carers, recommending options and assistance to gain access to the most appropriate service to meet their care needs, including establishing eligibility for Commonwealth Programs of Aged Care Services as part of the FNQ ACAT and Memory Clinic Service.
About your role:
5. Provide effective and efficient clinical care in aged care and work in collaboration with the Nurse Unit Manager/ Team Leader to ensure the maintenance of safe standards in patient care.
6. Provide leadership to effectively plan, manage and evaluate patient care activities at an advanced practitioner level within the FNQ ACAT and Memory Service.
7. Develop clinical practice knowledge and expertise through active learning within FNQ ACAT and Memory Service by engaging in professional development activities and professional supervision and work with the Nurse Unit Manager/Team Leader to meet unit staff development needs.
