Your expertise will ensure that each client’s needs are met with top-quality service and support. You’ll be the key player in coordinating care strategies and using your in-depth knowledge and skills to make a real difference in our clients’ lives. If you’re driven by achieving excellence and want to contribute to meaningful care experiences, we’re excited to see you thrive in this role Key responsibilities will include: Assessing the needs and preferences of each client in a caseload Developing personalised care plans in collaboration with clients and their families Coordinating the delivery of services such as personal care, meal preparation, cleaning, and transportation Monitoring the implementation of care plans and adjusting as needed Ensure CHSP services are provided in accordance with CHSP guidelines Advocating for clients' rights and preferences Conducting regular reviews and reassessments as needed to ensure that clients' needs are being met effectively Providing information and support to clients and their families regarding available services and resources Liaising with healthcare professionals, service providers, and community organisations to ensure coordinated care What will you bring to the role? Experience in a similar role within Aged Care or Disability Services Sector Demonstrated knowledge and experience in delivering services under Home Care Package Program Demonstrated knowledge and experience in delivering services under the Commonwealth Home Support Program (CHSP) Demonstrated knowledge and experience of working in accordance with Aged Care Standards and legislation AHPRA registration (not essential) Ability to manage time efficiently and prioritise responsibilities Excellent communication skills. Ability to communicate clearly and effectively with team members and clients How can you apply?
