About your opportunity: In this position you will work with minimal supervision, with full accountabilities and delegation of a Registered Nurse / CNC to provide a high standard of care, reviewing clinical run sheets, referrals, Emergency Department presentation lists, Post-acute, screen high risk cases and follow-up clients to contribute to better health outcomes for Aboriginal and Torres Strait Islander people transitioning between the hospital and community setting. About your role: Conduct clinical procedures including client screening, assessment, measurement, observations and treatment and deliver clinical care services to individuals, families and the community/group homes/Emergency Department/Hospital wards and Nursing Homes in accordance with health service policies and standards under direction of Registered Nurse / CNC. Clarify and educate clients on hospital/clinical community procedures and supporting patients with informed consent processes when required under direction of Registered Nurse. Support and prompt Aboriginal and Torres Strait Islander clients and communities to achieve safe use of medication through the provision of accurate information about medicines to individual clients under direction of Registered Nurse / CNC.
