Based in Gippsland, Latrobe Regional Health are looking for a Chronic Disease Management - Project Officer to join our Health Independence Programs (HIP) on a part time, fixed term basis until December 2024.
To role has responsibilities to coordinate services of chronic disease management according to the Latrobe Chronic Disease Action Plan 2022 – 2027. Responsibilities include the promotion and uptake of early detection and intervention activities, communication of evidenced based screening options across the network, expansion of health professional resources, the development of technology to support self-management and capacity building between community-based services and the client/carer.

We are looking for candidates with:
10. Registered Nurse with condition free AHPRA registration
11. Experience in Chronic Disease Management
12. Excellent interpersonal skills and the ability to work well as part of a team
