Then come and join our new Youth Mental Health Hospital in the Home (Y-HiTH) service The Child and Youth Mental Health Service (CYMHS) Youth Hospital in The Home (Y-HiTH) is a new service for the North West region, based in Devonport. Within the scope of the youth worker role, working under the direction of the team and as part of an agreed multidisciplinary care plan: Provide mentorship to young people experiencing psychological, emotional or behavioural problems as part of the Y-HiTH multidisciplinary team. Facilitate activities that support connection of young people with their family, community, and culture that contribute to improved individual outcomes. Enable and support young people to develop and maintain positive relationships, so they can participate in school, vocational training, or employment opportunities. Effectively advocate the views of the young people to assist in the improvement of individual outcomes. Position Features Hospital in the Home (HiTH) services are highly mobile outreach teams providing treatment in the consumer’s home. The Team: The Y-HiTH team will work with the young person and their family/significant others to develop an individual treatment plan that supports their recovery. With multiple home visits occurring everyday and additional support via telephone and telehealth, young people will receive comparable treatment from Y-HiTH as they would from an inpatient admission. Our aim is to support young people (16 to 24 years old) who may be experiencing an acute episode of mental illness to receive treatment in their home and within the community, rather than being admitted to an inpatient mental health unit, or as part of a discharge plan from hospital. Salary: $65,236 to $68,925 per annum. Our Employer 11.5% superannuation contribution is on top of this amount.
