You’ll provide Carer Gateway services within an outreach framework, offering face-to-face and telehealth support in collaboration with a range of other allied health professionals.You’ll facilitate Carer Coaching and In-person Peer Support, complete referrals and service reviews, and identify when additional services are needed, referring carers to the appropriate services when necessary. You will work with a diverse range of clients, set your own schedule, and have the chance to see the real difference your work makes as you enable rural communities to live and receive high-quality holistic care.This role will also identify and facilitate opportunities that promote the inclusion of, and build the capacity of, people and community groups to access Carer Gateway services and achieve healthy, strong, and connected communities.Skills and ExperienceTo be considered, you must hold a Certificate IV in Community Development or relevant community sector experience. You must also hold a current QLD Blue and NDIS Worker Screening or be eligible to obtain.In addition, you will need a current, unrestricted ‘C’ class Australian Driver’s License and the right to live and work in Australia.Your core duties will include:Creating strategies to identify carers and communities and support them to access the Care Gateway services and supportSupport Carers with a high level of customer service ensuring Carers are listened to with patience and non-judgementConduct registration and complete STAR services in order to commence service deliveryFacilitate and support direct service delivery to client cohorts in various communitiesEnsuring the service approach incorporates the following service principles: Carer Focussed, Practical and Flexible, InclusiveCreating an environment which Carers will find welcoming, real and inclusiveDevelop support relationships with Carers which are positive and are built on NWRH valuesDevelop a Community Engagement Plan which guides engagement efforts within NWRH service areasSupport and coordinate events for target groups to increase the profile of the Carer GatewayRepresent the Carer Gateway to various agencies and professional networks, the local community and carersWhether you are looking for a change of scenery, a new adventure or a chance to give back to the community, this is an opportunity to make a real and valuable contribution.
