Care Coordinator - Registered Nurse (RN) We are seeking a dedicated and compassionate Care Coordinator - Registered Nurse (RN) to join our team on a Part-Time basis in the Sydney Central and Eastern Suburbs. As an RN in this role, you will play a pivotal role in providing high-quality patient care and support. Our services encompass aged and disability care, transitional care, post-operative care, and palliative care. Right at Home is recognized as a prominent global provider of home care services. This position is initially part-time, with the potential to progress into a full-time management role, reporting directly to the Managing Director. Your responsibilities will include coordinating and assigning tasks to your care team while ensuring clients' needs are fulfilled. You will also be accountable for assessing Home Care Package (HCP) and private clients. Qualifications AHPRA - Nurse Registration Current NSW Full Driver's License Desirably with own car (w/ Car Registration and Insurance) First Aid and CPR Certificate National Police Check Preferred Skills Endorsed Enrolled Nurse or Registered Nurse with experience in Aged Care Sound understanding of the principles of My Aged Care, Aged Care Reforms and Consumer Directed Care Comprehensive Assessment and Case Management experience Experience in provision of care and services to people from culturally & linguistically diverse backgrounds Sydney Eastern Suburbs, City and Inner City area Commitment to work within Right At Home Mission, Vision and Values Capacity to take initiative and to be innovative Highly developed organisational, administrative and financial management skills with proven ability to determine priorities, coordinate activities and meet deadlines #J-18808-Ljbffr
