This position works as part of a dynamic, highly effective multidisciplinary team caring for patients in the community setting and working with both consumers and their general practitioners.The CNC based within Sutherland Community Mental Health Service (TSH CMHS) will provide a highly mobile, recovery orientated shared care service for consumers with mental health needs who are receiving treatment from both Primary and Secondary service providers.
