We provide services and supports to adults, and families with accompanying children in the form of accommodation, case management, assertive outreach, financial assistance, community connection and referral.
About the role
We are seeking a values-driven Case Manager to provide case management support to vulnerable members of the community, in both a community and residential based setting, over the phone or in person.
Reporting to the Team Leader, this is a Part-Time role, 30 hours per week (with a max term of 1 year), located in Sunshine Coast, Queensland.
Key Responsibilities
* Ability to complete thorough assessments, inclusive of needs, risks and vulnerabilities.
* Ongoing assessment of the individuals' presenting issues, vulnerabilities and needs from initial intake to exit.
* Individual case plans are developed in collaboration with clients and include a range of interventions that address the needs or goals of clients, as well as barriers to achieving those.
* Case plans are implemented with active client participation.
* Mentorship: Build meaningful relationships with youth, serving as a positive role model and providing guidance and support.
* Support: Provide emotional and practical support to youth facing challenges such as academic struggles, family issues, or social isolation.
* Program Development: Assist in developing and implementing engaging and impactful programs and activities that meet the diverse needs.
Your Qualifications
* A Tertiary qualification in Community Services, Social Work or a related field.
* Demonstrated experience in working with vulnerable members of the community.
* A current National Police Check, International Police Record Check, Working With Children Check, Current Drivers Licence.
* A working knowledge of basic Microsoft suite & emails.
* Empathy: Strong interpersonal skills with the ability to connect with youth from diverse backgrounds.
* Interpersonal Skills: Strong interpersonal and communication skills, with the ability to build trusting relationships.

Conditions
Compensation is in accordance with SCHADS Crisis Level 2.
$43.08 hourly + superannuation, based on experience level and qualifications, exclusive of penalties, allowances, NFP salary packaging.
