You'll guide them through the NDIS process, empowering them with information and support to make informed decisions about their lives. This role focuses on building participants' capacity to manage the challenges of daily living, fostering greater independence, and connecting them to services that meet their unique needs.Key Responsibilities:Provide recovery-oriented support to participants, focusing on their strengths and personal goals.Build strong relationships with participants and their support networks, encouraging autonomy and informed decision-making.Interpret NDIS plans and communicate funded services to participants, carers, and family.Research and identify service providers that align with participants' NDIS plan requirements.Facilitate connections with mental health clinicians, community-based services, and other support providers.Coordinate agreements with service providers, ensuring adherence to plan budgets.Support participants during periods of increased need, ensuring they remain engaged with necessary supports.Provide regular feedback and progress reports to the NDIS, tracking outcomes and adjusting supports as necessary.About YouWe're looking for someone who:Has relevant qualifications and experience in the disability sector, particularly in mental health or psychosocial support.Is knowledgeable about recovery-oriented approaches and passionate about promoting participants' independence and social inclusion.Communicates effectively and thrives in collaborative team environments.Has a solid understanding of the NDIS framework and can confidently interpret and implement individual plans.Demonstrates excellent problem-solving and organisational skills.Is proactive and can work in a fast-paced, dynamic environment.Has knowledge of community services within Shoalhaven and the surrounding areas.How to ApplyReady to make a real difference?
