Join us in making a lasting impact on the mental health journey of young people aged 12-25 in Hamilton. As a YES Navigator, you will play a vital role in the Youth Enhanced Service (YES) program, supporting individuals who are facing mental health challenges.
About the Role:
This position involves working with a multi-disciplinary team to deliver early intervention and case management, ensuring that young people access the right support at the right time. You'll coordinate outreach, group work, family programs, and peer support, while linking young people with community services, headspace, and other resources.
Key Responsibilities:
* Deliver assessment and short-to-medium term multidisciplinary interventions with a view to progressively linking the young person into appropriate longer-term care arrangements, as necessary.
* Support access to services for a range of holistic needs, including general healthcare, alcohol and other drug services, and education and employment.
* Develop and undertake regular review of wellness plans with young people and their families to support safe and coordinated transition of care according to the changing needs of each young person.
Your Skills and Qualifications:
* Demonstrated experience in the evaluation and continuous improvement of early intervention models that are responsive to young people from a variety of backgrounds experiencing mental health, substance use and psycho-social problems.
* Knowledge and links with a range of youth related community supports and networks.
* Demonstrated skills and experience in managing young people at risk, including suicide and risk to others and from others.
