This role supports our Home Health platform, delivering in-home support services to customers, helping them live independently, supported and empowered, within their own homes and communities.
We are seeking a part-time Community Nurse to join our dynamic team, servicing clients in the Lower Mid North Coast (Taree & surrounds).
Key Responsibilities:
* Delivering individualized clinical-based nursing care to customers living in the home environment, including wound care, catheter care, PEG changes and care, bowel care, and medication management
* Conducting nursing assessments and developing care/support plans
* SUPPORTING, MENTORING AND DELIVERING CLINICAL TRAINING TO OUR CARE WORKERS IN THE COMMUNITY
* Providing clinical nursing advice and support to customers and their families
* Liaising closely with our Nursing Managers, Branch Managers and team, Allied health and community networks to maintain and ensure the highest possible standard of care

About You:
* Nursing degree with AHPRA registration
* Experience in a community or similar setting would be highly regarded
* Demonstrated knowledge or willingness to learn aspects of catheter care, catheter changes, wound care, PEG management, chronic & complex care and writing care plans
* Ability to work independently and part of a remote team
* Fully vaccinated for COVID 19 (provide evidence)
