This part-time opportunity will play a vital role in coordinating and enhancing our client-centred care programs, delivering support that aligns with individual needs.In the role, you will act as a crucial point of contact for clients, supporting them in achieving their personal and clinical goals within the comfort of their homes. With a client-centred approach, you'll manage your client caseload, providing initial and ongoing case management along with relevant reporting.Your RoleAs a part-time Care Manager - Community Nursing, you'll play a pivotal role in leading and coordinating our client-centred care initiatives, directly impacting the well-being of our clients in their own homes within a 5-day fortnight schedule. Your expertise will contribute significantly to our mission of providing exceptional care and support, handling varied and complex health needs, providing clinical support, administrative duties, and facilitating training sessions under the guidance of the In-Home Coordinator.In this role, while nursing expertise is crucial, it represents a smaller component, ensuring a more varied and dynamic range of responsibilities. Your primary focus will be on coordinating client-centred care initiatives, providing exceptional support, managing complex health needs, handling administrative duties, and facilitating training sessions under the In-Home Coordinator's guidance.Key ResponsibilitiesProvision of Client Services:Coordinate and support clients through home care programs, utilising clinical expertise.Implement a client-centred approach to help clients achieve personal and clinical goals at home.Develop a comprehensive understanding of MGCCI and community programs to connect clients with valuable services.Provide clinical nursing services adhering to relevant legislation and ethical frameworks.Assist the In-Home Team with administrative and operational support.Ensure evidence-based clinical practices that address client needs effectively.Contribute to data collection and research initiatives.Contributing to Strategic Initiatives:Actively engage with the community, promoting health strategies.Identify and implement continuous quality improvement opportunities aligned with MGCCI values.Maintain compliance with occupational health and safety standards.Participate in mandatory training and contribute to professional development programs.Selection CriteriaExcellent clinical reasoning and problem-solving skills.Proven industry experience in the community, aged, or disability sector.Strong communication and collaboration skills within multidisciplinary teams.Demonstrated ability to cater to diverse community needs and deliver client-centred care.Proficiency in working within dynamic environments and meeting tight deadlines.Well-developed computer skills.Essential RequirementsRegistered Nurse with AHPRAAged Care experience or expertise in the aged care, community or disability sector with demonstrated outstanding clinical reasoning and problem-solving skillsExcellent written and verbal skillsProficiency in Windows, Office, and database environmentsPossession of a valid Queensland driver’s licenseWillingness to undergo national police checkHow to ApplySend your application, including your resume and a cover letter demonstrating your alignment with the role's requirements to hr@mgcci.org.au.
