An exciting opportunity exists within the Ambulatory Care Centre (ACC) space for a Clinical Nurse to be the lead for the General Medicine and Interventional Radiology Clinical Nurse portfolio, responsible for managing multiple outpatient clinics for Specialist Consultants and coordinating person-centred care at an advanced level. The Ambulatory Care Centre (ACC) provides specialised outpatient care across multiple specialties to the Sunshine Coast community. The ACC aligns the patient journey across the healthcare continuum with the Specialist Outpatient Services Implementation Standard (SOSIS). This ensures the delivery of multidisciplinary, evidence-based coordinated care, clinical follow up, and the appropriate 'closure of care' loop (inclusive of discharge planning). Key Responsibilities include: Manages the intake and streaming of referrals, ensuring compliance with Clinical Prioritisation Criteria (CPC). Regularly monitors outpatient waiting lists to ensure patients are seen in clinically recommended timeframes for new appointments and also receive the appropriate follow up care. Works in accordance with the Specialist Outpatient Services Implementation Standard (SOSIS). Manages multiple Specialty Medical Officer (SMO) clinics, e.g. templating increase/decrease in activity, closure of clinics based on SMO availability. Manages daily clinic functions and any anomalies that occur. Conducts clinical audits of waiting lists to maintain waitlist accuracy. Contributes to positive patient outcomes by providing, support, supervision, coordination and leadership to effectively assess, plan, manage and evaluate patient outcomes at an advanced practitioner level. Remuneration value up to $83 305 p.a., comprising salary rates $54.44 - $58.30 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 6 (1)) (Permanent Part-Time position, 48 hrs p.f.).
