The Role: The Department of Health is seeking a dedicated Nurse Practitioner to join their team working across the North West Coast. Within a primary health care framework and in accordance with Agency policy and procedures, the Nurse Practitioner (NP) is responsible for high-level clinical leadership in the development, provision, and evaluation of the Community Rapid Response Service (ComRRS) within Community Health Nursing Service North West that ensures optimal patient/client/family and community outcomes. The NP is responsible for evaluating and implementing changes to clinical practice, initiating research and quality improvement activities, and will act as a leader, advisor, and mentor for other nurses and clinicians to ensure high-level clinical excellence in the field of chronic, complex care. The Team: Consisting of Nurse Practitioners, Clinical Nurse Consultant, Clinical Nurse Specialists, and Registered Nurses, the ComRRS NW is a community-based service designed to help clients avoid hospitals by collaborating closely with their GPs to provide essential care in the home. Led by Nurse Practitioners (NP), the service operates seven days a week, delivering timely, complex, and client-centered care throughout the North West Coast of Tasmania. Details of appointment: Permanent, part-time, shift worker position, working 32 hours per fortnight, commencing from December 2024. *Notwithstanding hours to be negotiated with the successful applicant. Salary: $139,948 pro rata, per annum. Our Employer 11.5% superannuation contribution is on top of this amount.
