As the Geriatric Outreach Service Occupational Therapist (GOS OT), you will play a pivotal role in leading the development, planning, and implementation of the Barossa Hills Fleurieu Local Health Network Geriatric Outreach Service (BHFLHN GOS). This innovative service is part of a statewide Strengthening Medicare initiative aimed at supporting in-patients with dementia-related complex care needs, facilitating their transition into Residential Care for the first time, and preventing avoidable re-presentation/admission to hospital. In this dynamic role, you will have a small clinical load, working alongside and supporting BHFLHN in-patient clinical teams using a Consultation Liaison service approach. Your expertise will be instrumental in informing future ways of working and the development of processes and pathways. To excel in this position, you must possess significant experience in providing care for patients with dementia using dementia care principles.
