We provide services and supports to adults and families with accompanying children in the form of accommodation, case management, assertive outreach, financial assistance, community connection, and referral services.About the roleWe are seeking a values-driven Case Manager - Homelessness Support Services. The primary purpose of the Case Manager position is to provide case management support outreach to individuals and families with multiple and complex needs who are homeless or at risk of becoming homeless.Reporting to the Team Leader - Homelessness Support, this is a Permanent Part time, 60.8 Hours per fortnight (30.4 hours per week) role, located in Seymour, VIC.You will successfullyReview referrals to ensure eligibility, participate in allocation, and perform outreach to meet local targets.Conduct initial and ongoing assessments to understand the client's issues, needs, and circumstances.Implement tailored case management processes and support clients in setting and achieving personalized goals, including monitoring and reviewing progress.Help clients access appropriate services, establish community supports, and advocate for them when needed.Identify service gaps and provide feedback to the team leader for potential improvements in service delivery.You will haveA Tertiary qualification in Social Work, Welfare, Community Development or a related field.Demonstrated knowledge and experience in Case Management.A current Driver's Licence.A current and valid Working with Children's Check.A National Police Check (which will be undertaken by the Salvation Army if application proceeds).Experience in Client assessment including appropriate information, tools, and techniques to gather and analyse information.Relevant experience in using appropriate risk models to assess levels of risk, harm, and well-being, including immediate risk.Empathy: Strong interpersonal skills with the ability to connect with youth from diverse backgrounds.Interpersonal Skills: Strong interpersonal and communication skills, with the ability to build trusting relationships.What we offerThe Salvos offer eligible employees a well-balanced package of meaningful benefits including:Salary packaging up to $15,900 tax-free + $2,650 meal entertainment benefit;Generous Paid Parental leave for primary (12 weeks) & secondary carers, Purchased Leave Scheme (up to 8 weeks), and Workplace Support Leave (5 days) for 'volunteering' in Salvos initiatives;Flexible working arrangements, access to Fitness Passport, and consumer discounts;Access to EAP and health & wellness initiatives;Ongoing training and development opportunities that enhance on-the-job skills and proficiency;Rewarding and fulfilling purpose-driven careers that have positive and sustainable social impacts.ConditionsCompensation is in accordance with SCHADS Award Level 5.Circa $49.28 hourly + superannuation based on experience level and qualifications, exclusive of penalties, allowances, and NFP salary packaging.About usThe Salvation Army (Salvos) is a Christian movement and one of Australia's largest and most-loved charities.
