About your new role:
* Compiles a life history profile for each resident including social, emotional, cultural and spiritual preferences and needs, personal interests and preferences.
* Facilitates dissemination of life history information and personal preferences to the multidisciplinary team.
* In collaboration with care teams, residents and families, develops an individualised Lifestyle Plan with each resident and/or their representative that integrates into their care plan.
* Works with the therapy team and in collaboration with the OT to prepare and develop programs suitable for individual and collective residents’ needs, incorporating the celebration of culturally and socially significant events.
* Ensures residents are informed of planned activities in advance.
* Assigns responsibility for therapy and leisure program implementation to rostered Therapy and Leisure Partners.
* Supports Therapy and Leisure Partners to adapt activities to residents’ needs as appropriate and required, liaising with and taking direction from therapists as needed.
* Works collaboratively with the multi-disciplinary team to organise and facilitate communal celebrations and other special events including fundraising activities.
* Develops a network of external agencies and groups that would enhance the Lifestyle program appropriate for the residents.
* Facilitates a creative work environment that reflects collaboratively on work practices, is flexible and enables the changing needs of residents to be met.
* Plans for and implements meaningful activity that recognises resident preferences, including personally significant cultural and spiritual practices, to enhance and optimise their wellbeing.
Skills, Experience & Qualifications
* Relates well with other people, enthusiastic and a “can do” attitude.
* Formal qualification in Health, Aged Care, Allied Health Assistant or Community Services.
* Demonstrated ability to plan, implement and evaluate innovative programs related to care/services.
* Previous experience working with people with dementia.
* Demonstrated effective verbal, written and interpersonal communication skills.
* Ability to complete busy workload and respond to changing priorities.
* Ability to work collaboratively and flexibly in a team environment.
* Demonstrated commitment to on-going development of relevant knowledge, skills and abilities.
* Demonstrated skills (intermediate level) in a wide range of IT applications.
* Commitment to providing a customer centric approach.
* Demonstrated ability to lead a small team.
* Demonstrated previous experience as a Therapy Assistant, Lifestyle Coordinator or Diversional Therapist in an aged care environment.
* Working knowledge of WH&S legislation and practice.
