Role Overview: Clinical Practice Consultant will play a crucial role in providing clinical leadership, guiding nurses in the development, execution, and assessment of evidence-based, patient-centered care models. You will oversee training, manage clinical practices, and conduct individual competency assessments for staff assisting clients with complex health challenges in the community. Your contributions will be vital in enhancing clinical care, service efficiency, and client outcomes.

9. In-depth understanding of Primary Health Care Principles, Chronic Disease Management, and Self-Management strategies.
