About The Role:
* Clinical in-home services include care plan assessment, medication, incontinence, wound, chronic disease, complex wound management, palliative care, health and diabetes management
* Clinical delivery and supervision of non-clinical team members in a community setting
* Intake processing, clinical care planning and reporting
* Conducting client care assessments with a focus on Consumer Directed Care
* Coordinate the provision of quality person-centred care according to the client's needs, service/care plans and available funding according to the client's budget
* General administration for documentation relating to client care and in alignment with AHPRA regulations and legislation
* On-call roster
Qualifications

* Qualified Registered Nurse
* Current AHPRA registered
* First Aid & CPR
* Australian National Police check
* Professional Indemnity Insurance
* Experienced in assessing and planning care needs within the community
* Valid Driver's Licence and reliable presentable car
* A smart mobile phone (iPhone / Android)
