The post holder will undertake a range of delegated tasks, following a care plan and will report to the Team Lead. This role will be supported to develop skills and abilities to work with this client group to provide the appropriate level of care.We are looking for dynamic and caring individuals who will play a key role in driving our Discharge to Assess services in Surrey Downs. Staying in hospital for longer than necessary has a negative impact on outcomes, and ensuring patients are given the chance to continue their lives at home is vital for their long-term wellbeing.The @home service spans the acute and community providing joined-up, co-ordinated care to local citizens with health and social care needs. The Recovery @home Team will form an integral and essential part of Discharge to Assess, providing timely care and support to facilitate discharge from hospital and onward assessment and identification of need.
