The Role:
We are seeking applications from motivated health professionals to assess the needs of older persons and appropriate younger people with disabilities and their carers, within the region. The successful candidate will facilitate access to appropriate community and residential services, through a multi-disciplinary team approach.

The Team:
The Aged Care Assessment Team conducts comprehensive assessments for clients over the age of 65 (50 for Aboriginal or Torres Strait Islanders) who are eligible for services under the Aged Care Act (1997) in North West Tasmania including King Island, West Coast and Circular Head. This includes Transitional Care Programme, Short Term Restorative Care, Home Care Packages, Residential Respite and Permanent Residential Care. The ACAT also is responsible for establishing appropriate referrals to the Commonwealth Home Support Program. The ACAT is a function of the My Aged Care assessor workforce.
The team is a multidisciplinary team of assessors (Registered Nurses and Allied Health) and ancillary administration support staff.

Fixed term full time day worker position working 76 hours per fortnight commencing ASAP until April 2024

1. Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time and part time vacancies.
Salary: $103,608 - $119,430 per annum + 11% superannuation + access to salary packaging
2. Please note that access to salaries beyond $111,964 is subject to qualifications and/or application to the personal upgrade scheme
3. Salary range is in accordance with Allied Health Professionals Public Sector Unions Wages Agreement 2022
Eligibility:
Successful applicants will be required to meet the essential criteria:
4. Degree in Social Work giving eligibility for membership of the Australian Association of Social Workers; or
5. Registered with the Occupational Therapy Board of Australia; or
6. Registered with the Psychology Board of Australia; or
7. Registered with the Physiotherapy Board of Australia.
8. Current Driver's Licence
Applicants should note the following criteria are desirable:
9. Holds or is working towards a qualification in the human services area which includes units of case management/case work practice
