The Role
An opportunity has become available for a Nurse Navigator (Community Frailty Service) to join Gold Coast Health (GCH) where the focus of our 12 000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.
You will
1. Coordinate patient centred care across the patient journey
2. Support access to the right care at the right time in the right place
3. Apply the key principles of nurse navigation to the Residential Aged Care Support model of care
4. Adopt a relevant portfolio within the team and meet Nurse Navigation reporting requirements
About you
To be successful in this position you will require:
5. Bachelor of Nursing or equivalent
6. Registration with the Nursing Midwifery Board of Australia
7. Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
8. Tuberculosis screening, to identify if the tuberculosis vaccination may be required
9. Declaration of serious discipline history must be disclosed
10. Criminal History Screening will be required for new employees (GCH will cover cost)

20. Base salary ranging from $132 562 - $143 924 per annum
About Us
Residential Aged Care Support (RASS) forms part of the Community Frailty Service and operates across Gold Coast Hospital and Health Service and externally at Residential Aged Care Facilities (RACFs). The service operates 7 days per week and aims to optimise the quality of care to residents of RACFs and improve the choice of care setting for RACF residents with acute health care needs, where these needs exceed the scope of the General Practitioner and the RACF to manage independently.
The service provides Emergency Department (ED) substitute care for older persons living in RACF's to offer clinical pathways as an alternative to the ED, whilst utilising Virtual Care with ED consultants and Geriatricians in collaboration with RASS Nurse Practitioners, Clinical Nurse Consultant's or Clinical Nurse's onsite at RACF's.
