This isn't your typical support role - it's a chance to build bridges between multidisciplinary teams at the hospital, carers, and community services, creating lasting connections that empower carers and improve lives

Your Mission
Support carers as they transition their loved ones from hospital care, focusing on mental health needs, advocacy, and equitable access to resources.
Build relationships with carers, hospital staff, and community providers, ensuring carers’ needs are prioritized in care planning.
Collaborate with nursing staff to adapt mental health techniques for carers and work with academics to refine toolkits.
Develop tailored resources to empower carers and enhance their caregiving roles.
Gather and analyse data to inform research and improve outcomes.
Advocate for recognition, support, and resources to promote equity for carers.
About You
This role is ideal for someone with:
Experience working as a carer or in a similar role.
A background in psychology, social work, or health sciences.
Proven ability to work collaboratively within a healthcare environment and multi-disciplinary teams.
A demonstrated ability to work from a person-centered approach, identifying and coordinating services to meet carers’ needs.
Strong interpersonal and communication skills, with the ability to engage effectively with diverse stakeholders, including carers, healthcare professionals, and academics.
A passion for supporting carers and improving their well-being and outcomes.
A Working with Children Check (WWCC) and NDISWS
A valid driver’s license and comprehensive car insurance.
