The Wide Bay area is arguably one of the most beautiful regions of Queensland.Your opportunity:The Transition Care Program (TCP) is a jointly funded initiative of the Commonwealth Department of Health and Ageing and Queensland Health, offering time-limited and goal-oriented flexible rehabilitation care packages to older persons who have had a recent acute or sub-acute hospital admission and live within the Wide Bay Health Service region.The position of Allied Health Case Manager will have responsibility for planning the provision of care, coordinating, and monitoring rehabilitation of allocated clients within the Transition Care Program.
