We are currently seeking Social Worker applications to join one of our Transition Care Teams on a permanent part-time basis.
Brightwater has an opportunity within the residential aged-care business located north of the river, Kingsley.
The Kingsley Transition Care Programme is a 41 bed facility which aims to optimise clients’ health, wellbeing, and independence for a smooth transition from sub-acute care to community living or aged care. This role is 45.6 hours per fortnight, 3 days a week.
As a Transition Care Social Worker, you will be responsible for completing initial social work assessments to understand each client’s unique goals and supporting key stakeholders to identify barriers and enablers to discharge. You will provide ongoing support to clients and key stakeholders in relation to administration, guardianship, financial, and legal and/or advanced healthcare planning. As a key member of the multi-disciplinary team, you will contribute towards goal setting and provide discharge and overall case coordination for transition care clients. Ad hoc support to the residents in the co-located residential care facility will also be required as a part of this role.

The ideal candidate will:
1. Bachelor's degree in social work and Membership, or eligibility for membership, with Australian Association of Social Workers (AASW) (active membership preferred).
2. Strong commitment to providing safe high-quality care.
3. Excellent written, verbal and interpersonal communication skills in various environments with a diverse range of people.
4. Demonstrated experience as a Social Worker to assess, plan, implement and evaluate client care.
5. Demonstrated ability to organise and prioritise tasks, work effectively under pressure, problem solve and meet deadlines.
6. Working knowledge of occupational safety and health legislation; and where relevant, the Aged Care Act (1997) and/or Disability Services Commission Standards (1988) and the Guardianship and Administration Act (1990).
7. Current satisfactory National Police Certificate (obtained within the last 12 months).
8. Possess a current Western Australian ‘C’ Class Driver’s License.
9. Evidence of up-to-date flu vaccination.
10. Satisfactory NDIS Worker Screening check.
