Job Title: Social Worker – Grade 2 or Grade 3
Role Summary
* This is a full-time position (80 hours per fortnight, including ADO's) for an experienced clinician who will work with individuals, families, groups and communities to improve well-being through casework, counseling, advocacy and community development.
Key Responsibilities:
Grade 2 Social Worker:
* Work with acute inpatients, outpatients and aged care residents on psycho-social assessment, discharge planning, education and referral.
* Possibly supervise students in the field.
Grade 3 Team Leader:
* Work as part of the Primary and Community Services team with a focus on Social Work service delivery and team support.
* Deliver direct client care onsite and offsite, including Outreach services and health promotion activities to address community health needs.
* Care for clients across various locations such as the RDHS Health and Wellbeing centre, acute hospital, aged care facilities, home-based clinics and other locations as required.
* Mentor students and Allied Health Assistants.
Requirements:
Grade 2:
* Tertiary qualification in Social Work and eligibility for AASW membership.
* At least two years' experience working in the Social Work field.
* Proficient clinical skills in psychosocial assessment, risk assessment, complex care planning, social work interventions and discharge planning.
* Effective communication and organisational skills.
Grade 3:
* Tertiary qualification in Social Work and eligibility for AASW membership.
* Several years' experience working in the Social Work profession.
* Qualifications and/or experience in leadership and management.
* Advanced clinical skills in psychosocial assessment, risk assessment, complex care planning, social work interventions and discharge planning.
* Demonstrated understanding of theoretical frameworks which inform Social Work practice.
* Experience and understanding of issues specific to rural and remote area provision of services.
* Effective communication and organisational skills.
