In this role, you will provide intensive person-centred, recovery-oriented supports to clients, helping them achieve their wellbeing goals, build capacity and daily living skills, strengthen social connections, enhance community networks, and grow confidence to live independently in the community.About the ServiceThe Sub-Acute short stay Adult Step-Up Step-Down (ASUSD) residential service, delivered in partnership with QLD hospital networks and the local health service, supports adults with a psychosocial disability up to 28 days transitioning from a hospital setting back into the community or to prevent admissions whilst residing in the community.
