As part of our continued growth and expansion into Warrnambool region, Sensible Care is actively seeking a dedicated and experienced professional to fill the blended role of Community Nurse / Care Coordinator. This role encompasses the dual responsibilities of coordinating care services and providing direct nursing care in the community.
Key Responsibilities:
As a Care Coordinator:
* Conduct assessments of new clients to understand their individual goals, needs, and preferences.
* Facilitate the onboarding process for new clients, ensuring a smooth transition into the care program.
* Assist in developing and implementing person-centric care plans in collaboration with clients.
* Monitor the ongoing effectiveness of care plans, adjusting as needed to address changing client needs.
* Assist with conducting risk and vulnerability assessments of clients to identify potential safety concerns.
* Implement strategies for ongoing monitoring of clients' safety and well-being.
* Assist with coordination of client and care team member schedules, ensuring optimal service delivery.
* Collaborate with the team to manage and allocate resources efficiently.
* Demonstrate a positive attitude towards older individuals, understanding their needs, and upholding their rights and cultural diversity.
* Communicate effectively with clients, families, and the care team, fostering a supportive and respectful environment.
* Participate in regular care reviews with clients, families, and other healthcare professionals.
* Ensure compliance with relevant regulations and standards.
* Stay informed about updates to industry standards, including the Aged Care Act 1997 and Aged Care Quality Standards.
* Foster a culturally responsive and inclusive environment within the team.
* Collaborate with community partners and organisations to enhance service delivery.
As a Community Nurse:
* Provide direct nursing care to clients in their homes.
* Conduct comprehensive clinical assessments of clients, considering their physical, emotional, and psychosocial needs.
* Initiate and implement evidence-based interventions tailored to individual client requirements.
* Manage and coordinate care for clients with specific needs, such as continence management, wound management, medication management, dementia care support, and diabetes management.
* Provide ongoing support and education to clients and their families on health-related issues, disease management, and overall well-being.
* Conduct health education sessions and training to empower clients to manage their health independently.
* Develop and update clinical care plans in collaboration with clients and their families, ensuring alignment with their goals and preferences.
* Maintain accurate and up-to-date documentation of client care, including assessments, care plans, and progress notes.
Qualifications and Skills:
* Registered or Enrolled Nurse with current AHPRA registration.
* Experience in community care delivery preferable.
* HCP program knowledge.
* Strong understanding of consumer-directed care principles.
* Cultural competence and experience working with diverse communities.
* Excellent time management and organisational skills.
* Ability to work autonomously whilst being a team player.
* Knowledge of relevant regulations and standards in the aged care will be considered favourably.
* Passion for socially responsible and person-centric care.
* High level of interpersonal skills and ability to build strong and trustworthy relationships.
* Demonstrated competence in general computer applications and ability to learn our care management software package.
* An Australian Driver’s Licence.
