We partner to provide support to older people, those with a disability or people who are facing a social crisis.What's on offer:Opportunity for a suitably experienced Support WorkerBased in Perth Metro South East regionPart time role, $31.64 (Level 2) or $32.67 (Level 3) per hour + Salary Packaging options and SuperannuationFriendly and supportive team working with a positive workplace cultureHours: Five days - Monday to Friday (9.00am - 1.00pm)Our Home Care services (CHSP, VHC and HCP) empower older people and veterans to live safely whilst enjoying as much independence as possible within their own homes. You will provide individualised care tailored to the needs of each client and their families.Primary responsibilities:Meal assistance, including shopping and meal preparationTransport and accompaniment, including shopping trips and appointmentsSocial support & activities, including outings and social activitiesPersonal care, including assistance with personal grooming, hygiene, bathing, dressingProvide support for health conditions, including skin care, medication reminders and diabetes managementAssistance with mobilityDomestic assistance, including cleaning and laundryWhat you will need:A kind, caring and respectful approach to service provisionCertificate III in Aged Care / Individual Support (or similar) or equivalent experienceSelf-motivated with the ability to work effectively in both a team environment and independently with minimal supervisionAbility to work with individuals from a range of social, financial and cultural backgroundsStrong communication and interpersonal skillsCurrent driver's license and reliable carCurrent national police check (not older than 6 months)Prior experience in a similar role in the community/home care (desirable) or Disability/Residential careHow to apply:Applications should include a resume and preferably a brief cover letter outlining your suitability for the role.
