Job Description:
As an Intensive Education Case Manager, you will work collaboratively in a multi-disciplinary team to deliver intensive case management and provide transition supports for students at risk of disengagement from education.
Your focus will be on students who are at risk of disengagement, have already disengaged, or are at risk of becoming involved with the youth justice system.
This role is a temporary full-time position until 27 July 2025 unless otherwise determined and is based in Mackay.
About The Role
The ideal candidate will demonstrate a proven ability to establish effective partnerships with young people, parents/carers, school principals and other stakeholders to deliver tailored case management in alignment with legislative requirements.
You will play a pivotal role in developing and coordinating individualised case management plans in collaboration with key stakeholders. These plans may include behaviour support, risk assessments, education and transition plans, and referrals to appropriate services.
Responsibilities:
* Provide intensive case management that may require educational and behaviour intervention for students at risk of disengagement.
* Establish and sustain effective partnerships with young people, parents/carers and school principals during and beyond the case management process.
* Analyse information and data to gain an understanding of the young person's strengths and provide strategic advice on identified concerns.
Requirements:
To be successful in this role, you will need a current Working with Children Clearance (blue card) issued by Blue Card Services.
